General COVID-19 guidance for members shooting VAGUE productions. During the Coronavirus pandemic the health and wellbeing of our crew, cast and visitors is our number one priority. We can all help control the virus if we all stay alert and follow government guidance.

Due to the current coronavirus (COVID-19) pandemic, we require all cast, crew, contractors and visitors to complete this self-declaration and reporting form for all in-person activity. This form should be filled within two days (no more than 48 hours) before your arrival at the location.

Before you host, speak or star in a VAGUE production, we need you to sign this release form to make sure you're fully happy with us using your likeness and the relevant information you provide in this project.
